This life insurance policy guarantees a benefit upon death and offers a long-term care (LTC) rider with two kinds of assistance if needed. The LTC benefit is provided either in a nursing or other care facility, or at home:

• Custodial care: Support with personal everyday tasks, like bathing, eating or going to the bathroom
• Skilled care: Support from a medical professional, like a nurse or therapist

This supplemental coverage can help cover care you or a loved one may need beyond the short-term skilled or custodial care your major medical or Medicare benefits provide. And the life insurance covers you in the event of death.

To enroll, visit allstate.benselect.com/bcu. The first time you visit, you will create a personal identification number (PIN) using the last four digits of your EIN (Employee ID number) and your full birth year. For example, if your EIN is 12345678 and your birthday is Jan. 1, 1980, your PIN will be 56781980.
