This site and any accompanying inserts provide highlights of your BCU benefits for the 2025 plan year. This information is not your summary plan description, which will be provided in follow-up. HIPAA Privacy Notice
Under the Health Insurance Portability and Accountability Act of 1996 (HIPAA), BCU’s health plans are required to provide you with a HIPAA Notice of Privacy Practices (“Notice”) at the time of your enrollment in the health plans, and at certain other times. In addition, the health plans are required to periodically notify you of the availability of the Notice and provide you with information on how to obtain a copy of the Notice. A copy of the Notice for BCU’s health plans is available here. You may also request a copy of this Notice by contacting the BCU Benefits Team. If you participate in insured group health plans sponsored by BCU, you may also receive separate Notices from our insurers. To the extent that the applicable health plan contains benefits other than those covered under HIPAA’s privacy rule, this information pertains only to those health care benefits that are covered under HIPAA’s privacy rules.
Women’s Health and Cancer Rights Act of 1998
The Women’s Health and Cancer Rights Act of 1998 requires BCU to advise you annually of certain benefits for mastectomy-related services. For individuals receiving mastectomy-related services under a BCU medical plan, coverage will be provided in a manner determined in consultation with the attending physician and the patient for:
(a) All stages of reconstruction of the breast on which the mastectomy was performed;
(b) Surgery and reconstruction of the other breast to produce a symmetrical appearance;
(c) Prostheses; and
(d) Treatment of physical complications of the mastectomy, including lymphedema.
These benefits will be provided subject to the same deductibles and coinsurance applicable to other medical and surgical benefits provided under the applicable BCU medical plan. For more information, consult Aetna Member Services (on your ID card) or contact the BCU Benefits Team.
Notice of Special Enrollment Rights—Including Under Your State’s Children’s Health Insurance Program
Under HIPAA, you have the right to enroll in BCU health plans at times other than the open enrollment period, upon the occurrence of specified events. These enrollment periods are known as “special enrollment” opportunities.
Loss of Other Coverage: If you decline enrollment for yourself or your dependents (including your spouse) because of other health insurance or group health plan coverage, you may be able to enroll yourself and your dependents in BCU health plans if you or your dependents lose eligibility for that other coverage (or if the employer stops contributing toward your or your dependents’ other coverage). However, you must request enrollment within 31 days after your or your dependents’ other coverage ends (or after the employer stops contributing toward the other coverage).
Obtaining a New Dependent By Marriage, Birth, Adoption, or Placement for Adoption: If you have a new dependent as a result of marriage, birth, adoption, or placement for adoption, you may be able to enroll yourself and your dependents. However, you must request enrollment within 31 days after the marriage and within 60 days after the birth, adoption, or placement for adoption.
Eligibility for Assistance Under Medicaid or a State Children’s Health Plan: If you or your dependents (including your spouse) become eligible for assistance under Medicaid or a state children’s health insurance program, you may be allowed to enroll yourself and your dependents in certain BCU health plans if you are eligible for coverage under the applicable BCU health plan terms but not enrolled. However, you must request coverage under the applicable BCU health plans not later than 60 days after the date that you or your dependent is determined to be eligible for assistance.
Termination of Coverage Under Medicaid or a State Children’s Health Plan: If you or your dependents (including your spouse) are covered under Medicaid or a state children’s health insurance program and the coverage is terminated because you or your dependents lose eligibility for the coverage, you may be allowed to enroll yourself and your dependents in certain BCU health plans if you are eligible for coverage under the applicable BCU health plan terms but not enrolled. However, you must request coverage under the applicable BCU health plans not later than 60 days after the date of termination of the Medicaid or state children’s health insurance program coverage.
